Catalogue item being auto approved when the approver changes.

We have had a catalogue item which has been approved automatically.  There is one person in the approval group who changed after that item created.  How can we prevent this?

By chance: was the auto approval done at the same time this swap took place? Was the ticket already in the flow when that happened?
