Every change goes to the Review state, regardless of its Close code.
If you want different behavior, you can add a step in the Flow—for example:
If the Close code is "Unsuccessful", then create a task in the Review state.
If my response proves useful, please indicate its helpfulness by selecting " Accept as Solution" and " Helpful." This action benefits both the community and me.

ServiceNow's Change Management process, there is a default behavior where changes with certain Close Codes (like "Unsuccessful" or "Successful with issues") are all expected to go into a Review state.
However can you explain what is happening in your scenario when you are adding Close Codes (like "Unsuccessful" or "Successful with issues") in detail.

I think there might be some misunderstanding. As you can see in the flow designer, every change goes to the Review state, which I have already mentioned as well.
By seeing flow, you can take the action.
