If we want to be able to create change tickets linked to a request does any one know if this should be done a the request, requested item, or task level? What would be most in line with ITIL? (I think the request is too high level so probably the requested item or the task...

According to what you're also stating, the correct thing would be requested item. Requests can contain multiple requested items which again can contain multiple tasks.
