Demand form - 5 column field section - is it possible?

I'm working on updating your demand form and one of the requirements is to include a section with 5 columns. Something similar to a variable set on a catalog item. Is that possible? I can't seem to figure out how to get more than two columns. See screenshot for an example. If this is possible, please tell me how.

I guess you are talking about MRVS. That is something you can have on the portal forms, but not on the forms in the backend (not in Core view and not in Next Experience).
If you create a different table for that data, you can add it as a list and have people fill it through list edit, but that depends on the requirement.
Forms in the backend (just like on the portal) can only contain 2 columns. MRVS is a different component which is just related variables in a row. It's not a true 'form'.

Hi Mark, Thanks for the reply! I'm familiar with how do to do this on a portal form, and was hoping there was a similar. I'm not familiar with MRVS. What is that?
