i have below scenario how we can achieve this please suggest me.
We currently have ‘UAR Quarters’ which are used to created User Access Reviews based on a simple filter that runs against UAR templates. We want to update this filter to be more dynamic by adding a condition builder on the UAR quarter form that utilizes the fields from the UAR template form and updating our flow to utilize the condition builder.

If it is the top task related fields you want to show then you can use below else you have to provide your requirement in detail like table name and in the export defintion condition builder which field has association with UAR etc.

we have UAR quarter form used to create User Access Review below screen shots u can refer, after creation of UAR quarter form we have to create the User Access Review record there we have to select UAR template both screen shots added below
