I have a user that is trying to unsubscribe from this notification but cannot due to this error code. How can I diagnose and resolve this issue?

Refer KB: KB2582963 Duplicate Custom Notification Subscription Fails with "Unable to Update Preferences" Error
Resolution
Review existing subscriptions in the sys_notif_subscription table to confirm whether a subscription already exists for the notification.
Instead of creating a new subscription, modify the existing one to update filters or delivery channels as required.
This is expected system behaviour as per ServiceNow’s notification subscription design
