How to create a report for this below requirement?

1.Average number of Approvals per Change

2.Highest number of Approvals per Change 

it’s possible for PA please explain?

Yes @Shashi K with PA it is possible but per change will be too much for you. Try to add parameter per month / week to ease the analysis.

For average number of approvals pr. change this is defiantly possible. Here you can do the calculation in a formula indicator. For the highest number of approvals per change, I don't fully get your requirement here, but I do think you need a scripted automated indicator to do this.
