How to create QR Code / Barcode to submit an incident with Now app

USE CASE: Reporting a non-IT incident with a mobile application (Example: water fountain incident), using a mobile application (a QR code is pasted on each piece of equipment to facilitate incident reporting). This QR code is to be used to report the incident)
