How to display details of request on Survey

Hi Team,

We have survey created on sc_request table, which triggers when request is completed .

Currently it only displays questions we have created in the survey. 

How can we include details of the request i.e. RITM number, catalog item name, description

I would make the survey at the RITM (sc_req_item) level instead of request. That way out of the box it will display the Number and item name. You would need to due some customizations to get the request to display the ritm number and catalog item name. This would cause technical dept especially if you are using the service portal as you would have to modify the out of the box widget.
