How to read attachment value in Reports?

We have a requirement to read attachment values on report.

 

So, after submitting a request for catalog item, we have to read the values in attachment on report

 

Can Anyone let me know how to achieve this?

So you are you saying you want to include values from an attachment and include them in a report? I'm not sure that is possible. Reports can only show fields in a table.
