Can someone help me to understand how to get the requirements from clients for Knowledge Management Application (ITSM). 

It would be great if you could share a template as well.

There is no standard template you will get for requirements gathering as it vary from company to company. What best I can suggest 

Go to Now Create

Nowlearning.servicenow.com/nowcreate

Go to Asset

Search for knowledge management 

You will get few docs like process doc, starter stories and use that to build your template.

Believe me you will not regret if you visit noe create once.
