Pre Approval Request for change

Hello ppl , I have a requirement to create a change template

scenario is, we have a change process for repeated changes which will be going to managers approval and so on.As some of these changes would be frequent changes we are creating a RITM where we will be taking the required fields for change in Request once the request is approved we want to create a change template which will be auto fill from Request. can some one suggest how to implement that ?I can pull the data from RITM but not able to insert in to template.

Thanks for the reply. But once i create the Request it will be filling from the Request to new template and it will sent to the user . from there he will create the change using that template many as he wants
