Request not found when submitting support request from record producer via Service Portal

First I hope everyone is faring well during this global event.

I have record produce which is pretty much the out of the box to submit the initial incident request for support.

Lately, when a user submits this via Service Portal they will see the following( image  1) instead of the Generated Task Record as shown in the record producer form. Also not it shows Order Statu at top.

Just some guidance on what to look for to resolve this.

How did you go with this, ever get it resolved? We're having the same problem atm. It's random, happens to any catalog item, leaves no errors in the logs, and can't be replicated. As a result we're finding it hard to troubleshoot!
