Risk assessment is not showing questions when open

When you select the Related Link to go to the Risk Assessment, a window opens, but no questions are listed. This is working on the TEST and PROD instance, but no longer working on DEV. Trying to track down what changed on DEV to cause this issue. Need some suggestions of what to check/verify.

Below is the response I received from SN (but as I recall there was another issue that was causing the problem - thought I wrote the solution down, but cannot find it, I will keep looking and share if I find it):
Issue Summary: Risk Assessement not opening correctly in DEV instant
Reason: Dictionary default display value changed for Change Request
Solution Proposed: The reason you are facing this issue is because you have configured the dictionary Display for change_request table to be “Reason” instead of number. Please revert the change by checking the “Display” flag on this dictionary.

PRB1413792 is being worked on by dev “Blank Assessment presented, with NullPointerException in logs when record being assessed has an empty field set as its dictionary display”
