Risk Assessment not Authorized

Getting an unusual error.
I have several Risk Assessments that are defined and I know work another instance.
However in this particular instance when I click on Fill out Risk Assessment in the Change, it gives me an error saying "You are not authorized for this assessment"
I cannot work out why this is not allowing me to execute it. Even Admin cannot run any assessments.

Simply navigate to System UI->UI Pages. From the list of UI pages, right-click on the list header and choose "Import XML." Then browse for the xml file and import it and you're done.

Right on. It's probably also worth mentioning that you can do this from a completely unrelated table. A UI page XML could be imported from an incident list (or any other list in the system) for example.

I figured that out last night when I was playing with it and seems to work perfectly soon as I can figure out how to weight it to use it. At least my form is right now I just need to get the conditions to populate on there and figure out what they want. Our company didn't want a Priority state of 5 for Incident or Change so that's been a real treat trying to figure out how to get it now display that value. I finally found the last business rule that was getting it to display for incident. I think i seen it on change last night when I was playing with that so now on the search for that one...
