The state remains as "Requested" for Test Approver 1 even after it has already been approved. When the second approver (Test Approver 2) approves, the state of Test Approver 1 resets back to "Requested.
What operation are you performing in update ritm record after every approval ? Ask for approval is of which type like anyone can approve or all approve, % people can approve?
Use the Set field/column of Table: sys_history_line (All > sys_history_line.FILTER) will help to identify any such record history as what all happened with the record. I would recommend you look at it to identify whether any existing server-side logic is recreating the approval records or not.
