Does anyone have any great examples of presenting end users with a form to complete for them to request a new server? Seems quite clunky and long winded to build something, so looking for ideas.

By requesting a new server do you mean VMware or EC2 servers as mentioned below or something else?

Hi Tony,

I'm just looking for an example or a template I can import that uses a record producer to create an incident. I'd like the example to capture the typical things you want to know about a server.

I find building the form using a record producer to be very clunky and am hoping someone's already done it well.
