Service portal for dummies - how to add 'description' field on standard form

in short - 

our basic form in SP shows incident number, priority, state, affected user, assignment group, assigned to, and short description.

i need to add description...but can't figure out how. i know the widget being displayed is just "form" but can't figure out how/what to add to display a simple field on the standard UI.

Click on the hamburger ,(three lines) top left corner of the form . It will allow you to edit the layout and you can add description field to your form. 
Create a service portal view (best practice).
