Set the field values from url on a form

I have a UI Action which when clicked redirects to a new form and now I want to set the values from the url to the fields for that I have written the UI action script for redirect and setting the url and one client script for setting values from url to fields but it is not working properly. It is giving me the values in url but it didn't populate it in the fields.
Can anyone help in this?

I am unable to view the content of your attachments - try pasting in screenshots instead.  Are you saying that you are getting expected values in a log/alert in the Client Script? which field(s) on which table/form are you attempting to populate what value(s)?
