I got a requirement in which I was asked to create standard change templates. I created the templates in the Developer Instance and moved to Test Instance. But while creating Std Change Record with the specific Templates. the Values are not getting displayed on the form. I guess it might has some proposals or versions related issue that i am able to figure out. Can Anyone Help me with what I am Missing?

Did you verify before migrating if everything was captured in your update set properly?
Since the template is not moved properly to TEST the field values are not getting populated on your CHG.

Did you move it using Update set.
Check the version and check whether it is active.

It should work for you. I’m assuming it’s fully approved and you’re using it. Please check whether there are any ACL-related issues or user permissions (for change manager or ITIL user).
