update set captures action as delete when adding choice

Hi All,

I'm creating new Choices(sys_choice) table

Table: Incident 
Element: Urgency
Label and Value as well. 

I'm having trouble with the update set capture the information.

It captures as "Action: DELETE".

I've attached the screenshot for reference.

Someone please help!
