Issue with the "New" button in Configurable workspace

Hello all,  I want to see if anyone has gone through this and can assist.  I made some changes in UI Builder to the Service Operations Workspace in our dev and test instance the "New" button works fine and opens a new incident form.
But when I deployed to production, while all my changes work, the "new" but does nothing

I've checked to see if my "New" button is configured correctly and it seems to be since I really didnt do anything with the "New" button

Figured out that my update set had not captured the changes to the UX ad-on Event Mapping - Service desk landing page overview SNC Service desk landing page overview SNC SOW overview UXF List New map - polaris variant
