What are the possible reason for duplicate of role in selected slush bucket?

Hello experts,
Do you know of any possible reason for the having a duplicate of role in the selected slush bucket? Please see the image below for the reference:
I tried testing it in my PDI by creating new record for the 'User Roles' table but as you can see it doesn't create a duplicate.

did you check the sysIds are same for both the records?

Hello,
Yes I have checked the records in the User Roles table. It seems that they have different sys_ids.
Do you have any possible idea how was it created?
When I checked on the created date, it seems it was created at the same time:
When I am trying to create a duplicate record in User Role table in PDI, it is not working.
P.S. I don't think it is possible to export xml in User Table since it is a bunch of reference field which open to user record. Do you have any inputs?
