Single Selection Checkbox for variables on Request Item

I've created a catalog item for one of our departments.  On the form I have created a variable set to accomplish single selection with check boxes.  From the service portal the checkboxes work as expected, only one can be selected at a time.  If I want to select something different I have to uncheck the current selection first.  But if I navigate to the request item table when the same form loads I see the field selected but the other options are not read only as they are in the service Portal.  I have created UI policies but it's still not working.

I have 3 UI Policies like the one I posted.  So when one is selected the others are read only.
