12 month rolling overall health status

I am looking to create a dashboard grid that displays the overall health of projects based on the most recent status report for each month. Attached is an example of the desired output.

My goal is to achieve this using out-of-the-box functionality wherever possible, with minimal custom development. Ideally, I would like the solution to be built using a Data Visualization.

Have you started by looking into which functionality you have available and how you would like to a hive it? It sounds like a report that you have en excel that you would like to build in ServiceNow, but you have no idea about which capabilities there is.
