Demand Assessment Design

I am looking to add the description of the Question to the assessment itself but I can't for the life of me find the design of the assessment.  I am just looking to use the OOB questions to start.

Example:  
Strategic Alignment section:

Description is:
Please score each question on a scale of 1-10
[10 = "very much so" & 1 = "not at all"]

However, it's nowhere to be found on the actual assessment, and I can't find where to edit the design of the assessment.

Hi @Community Alums ,

Navigate to All > Project Administration > Settings > Assessments Metric Categories.

Select the Strategic Alignment section within the assessment metric category 

Make the required changes on the Mertic Form and update.

But how do I get that on the actual assessment that someone fills out??

Hi @Community Alums 

1)  The demand state should be Qualified and the stakeholder must have the Assessment Required Checkbox checked

2)  Naviagte to  Self-Service > My Assessments & Surveys.

3) Select the assigned Assessment  to review
