I have a requirement to create a new field in status report in project workspace and field should appear if health is yellow or red..

>> Create a UI Policy:
Table: Select the relevant table for your status report.
Condition: Set the condition to Health is yellow or red.
>> Define UI Policy Actions:
Field: Choose the new field you want to make mandatory.
Mandatory: Set this to True.
This will ensure that the new field becomes mandatory only when the Health field is set to yellow or red. ...

Status report is launched via UI action, go to below instance URL for UI action
https://<instance>.service-now.com/sys_ui_action.do?sys_id=ce4427749f371200598a5bb0657fcf5b
Add the conditional fields from UI Action script, you can use below post for reference
