I’m a BA and BPC, and this is part of my day-to-day work. Practically speaking, there isn’t a standard template for requirement gathering because requirements vary for each module.
However, there’s a helpful post by Bita that you might want to check out. That said, I’d still recommend following the Now Create assets and conducting process workshops. During these workshops, you can start gathering requirements, as the workshop decks provide insights into the "Out of the Box" (OOTB) processes. This helps you understand what’s already available and what needs to be customized.
You can also leverage starter stories to support requirement gathering. Note that ServiceNow doesn’t provide a standard requirement gathering document.
