I have a current catalog item and already build the workflow.
The stakeholder requested that if the request reject on the first time, it will go back to the requestor asking for additional information/attachment. Once the user add a comment and update the form, it will push for approval again.
My question is, how can I add the additional information that the user input in the comment or the new attachment to the approval again?
Because right, when the request is push to the approval, the new comment and attachment are not included.

@Merza Lyn , We had a similar requirement in one of our catalog items.
For the rejection , I would recommend you to create a catalog task for the requestor to send all the additional information/ attachment. (You can copy the comments and attachments to the RITM that were added on the sc task by the requestor). Once, the sc task is closed , you can send back the same approval one more time.
This approach will help you to better track how many times the approval was rejected , task was created, information was entered.
If you are doing it using flow designer, you can create a subflow if there are multiple approvals invovled.
Doing this worflow editor is also achievable , just you need to join the activities.
