The validation gets failed in SNPI Story 2 - Configure List and Forms

Kindly help me resolve the issue where i have configured hhd app and table and form table creation task got validated successfully but form layout task is failing i have tried recreating state field but no luck also have tried deleting the app and recreated it but the task is not validating successfully and i am unable to complete next tasks and i dont have any extensions/instances available for my training instance of cis implementation australia..tomorrow is the last day for my  lab instance  kindly help me solve the issue
