if yes then that Reason must be within that UI page

Check your Cancel Change request UI action where g_modal has been used to show the pop-up and popup window has been designed in the UI page . UI page name also you will get from your code.

Gi To naviagation, type UI page-> open "change_confirm_reason" UI page , there you will get Reason field is there as INPUT
