What is the best business practice to create Create servicenow catalog item with six main questions, each parent has a child Selectbox questions?

  4. Option 4: Graduate Program Readers Account Creation

Each request type has the following  Select Box  questions

 

When Request Types = Option 1: Request New Program

 

Program Name – Single-line Text – Mandatory
Help text: “Please give the full name of the program you wish to create a supplemental Slate application for. Please use the complete and full program name, as listed on CalState Apply.”

Replacing Existing Program? – Single-line Text – Optional
Help text: “Is this new program request going to be replacing an existing program in Slate? If yes, please provide the old program name and old program code that the new program will be replacing in Slate.
Please note that the old program entered here will be inactivated within Slate. Please provide an effective date for this new program, on which this new program can go live in Slate, and the date on which the old program can be deactivated on (i.e. the date on which there will no longer be any pending applications tied to the old program code in the pipeline, which could be negatively impacted if the old code were deactivated).”

Required Program Materials – Checkbox Group (multi-select) – Mandatory
