I need to create a simple form where users can enter data.  The form need about 24 fields and allows inputs to be saved to a table.

Hi Aman

I am not interested in the ServiceNow Form Designer.  I mentioned the UI designer and the input component.  Consider the Document Approval Template.  This application works like a regular website with pages on a web server.  The ServiceNow forms are not suitable for my customers.   Is there anyone at ServiceNow that I speak with?

Consider the Document Approval Template except I am not approving documents, I am taking data from input boxes.  I hope this makes sense.
