item has appearing duplicate attachment on esc portal

Hi Everyone,
On esc portal when i'm searching item it appear duplicate attachment with same name as i highlighted in green color  in below screenshot:

how to fix it can anyone suggest it?

hi @Ankur Bawiskar  - No, there is no attachment attach to the corresponding catalog item as you can see in below screenshot.

Is it behaving the same for other items?

was this working fine earlier?

Did you recently upgrade?

Try raising a case with ServiceNow.

1)Is it behaving the same for other items?

   Ans: yes, but not for all item for some item it is behaving like this

2)was this working fine earlier?
  Ans: Expected behavior is in dev but issue is happening only  in "Test" and "Prod".
3)Did you recently upgrade?

Ans: No
