Request item fields are empty in sc_task table

I see the request item fields are suddenly empty in the sc_task table. I checked the business rules and client scripts to see if there are any operations affecting the sc_task table. Unable to identify the root cause. Can someone help me on this.

Couple of questions. 

Were all Requested Item fields populated?

Is this in a Prod or sub - prod environment?

Can you confirm the Item that should be in one of those fields exists in the instance?

If they were moved from environment, say dev, to another, say test, the items may not have been carried over. If they had values prior and no longer do, it may be a case of broken references.
