CI Group approval history details are getting updated twice on change request

Hope you are doing great!.

We have upgrade our instance from Vancouver to Xanadu, After upgrade onwards CI Group approval history details are getting updated twice on respective change request.

Any kind of help is really appreciate it.

Your approvals are set by workflows, flows or how?

Perhaps the upgrade created approval logics that has been duplicated, check if it cannot be the case as there is a few seconds difference between the set of two approvals..
