I wanted to check if anyone has received the prize confirmation email for the Phase 2 results of the Lift Off to Knowledge challenge (ServiceNow hat and notebook).

Has anyone here received the email or prize confirmation for Phase 2?

'[...] ELIGIBILITY: The ServiceNow University Lift Off to Knowledge Sweepstakes (the “Sweepstakes”) is open

to (i) individuals except those who are legal residents of the Province of Quebec (Canada), Egypt,

Argentina, Belgium, Hong Kong, Singapore, Thailand, Italy, Mexico, Brazil, Dominican Republic,

Philippines, Australia, Finland, Israel, Pakistan, Poland, Romania, Uruguay, Colombia, Austria, Denmark,

Hungary, Luxembourg, Nigeria, Portugal, Saudi Arabia, South Korea, United Arab Emirates, Turkey, and

Morocco (“Ineligible Countries”); (ii) individuals who are the age of majority or older in his or her

jurisdiction of residence at the time of entry. [...]'

Just follow the steps in the second mail and you're good to go.

I have got Entry Period 1 prize mail form outreach@rtm.com but I have one confusion in filling the W8BEN form. I'm an Indian Resident, so which form I should submit is it W8BENE or W8BEN? 
And in W8BEN form what I have to fill in Part 2 Claim of Tax Treaty Benefits question 9 and 10?
