While clicking on pending button the state is not getting changed to pending in problem form

In the problem form when the state is in root cause analysis. If I click on the pending button and fill the mandatory details, it then click again on pending button the state is not getting saved. When I refresh the page it goes to root cause analysis state again.

 Is it  OOTB UI action, i cant see in my PDI.

Might be issue with UI action code.
