CHG interceptor is being bypassed from SCTASK

We realized that our SOW plugin was very out of date so we updated it over the weekend after testing in the non-prod instances.  I apparently forgot to test all change types because we are having an issue now that I cannot seem to resolve.

From SCTASK, we have a related list for Change Request.  Our teams click the New button to go to the interceptor page to select which type of change they are wanting to open.  Starting after this update, it is going straight to a Normal Change instead of the interceptor page, as though Normal change is the default.  It is still working normally in Core UI so I assume there is something minor that I need to check and switch back. I am not super familiar with the SOW side of things so I do not know where I should be looking to revert this back to the original functionality.  

I would add screenshots but I don't even know what kinds of screenshots would be needed - so please let me know and I can grab anything.  Any assistance would be greatly appreciated.

Please check the Declarative actions from the left nav, Usually the the buttons of SOW (form actions, list actions and related is actions) are configured through Declarative actions.
