Incidents and Requests not creating

Hi sometimes when I create a case and within that create an incident and a request it hasn't created the incident or request and remained assigned to me could someone help with this. Jenifer

We may need more information to answer your query. When you say 'Create an Incident' or 'Request,' are these UI actions/buttons on the Case form?
