State field change should reflect in activity log along with additional comments

Hi, I have a task.
When we change the state in SC task for example when we change state work in progress to pending then the comments will be visible in the activity log of RITM Form like ‘comments of pending state along with added comment. this should be replicate in the RITM form activity logs.
example: state is pending, and comment is so and so.
I request you to help me how to do this.

To add state and additional comments fields to activity log > click on the funnel icon next to activities and configure available fields. Add fields of your choice

Actually I want in RITM additional comment the state should be mentioned like. Example Additional comment: Sctask Number -pending- "the example comment" in single sentence as a comment.

this is an easy requirement, what did you start with and where are you stuck?
1) after update business rule on sc_task condition as State Changes
