Activity stream Showing the Different name instead of system when need attention updated by user

Hello Everyone,
I am facing this issue from last few days.
Could you please have a look and provide me the better solution for this.
Issue: When user change the need attention field that time system puts his comment as "action status needs attention was empty" but for me instead of "system" different name is showing there.
Workaround:
I have checked the UI action , Flows, sub flows, business rule, client script, script include, UI policies to see is there anything written to alter this functionality but i didn't find anything.
ServiceNow have given the solution to clear the cache. So as per the ServiceNow suggestion i did it.
What do you think Will it occur again? If it occurs then how i can resolve this issue?

are you the one who is working on that case?
Possibly OOB the system checks this and uses that, but ideally it should take the logged in user name

I am working on that case but when i update the need attention check box system puts his comment . But for me instead of system showing different name.
