Public Survey with available Case record number and contact in Assessment record

Hi Snow Community,

 

Im trying to have a PUBLIC survey that also gathers the Case ticket number and Contact information without asking to login,

This Public Survey will be triggered 100% of the times when a Case ticket is Resolved,

I already have the Email notification created with the Survey Link in it, Survey already marked as Public,

What we want is Customer should not need to login in ServicePortal to complete Public Survey and also we need to have any type of configuration to gather Customer Case ticket (From were this Survey was triggered) and information from that Customer (Contact assigned to Case ticket).

So Whenever Customer clicks on the email hyperlink that points into the Public Survey, then after completing simple answers and hitting in SUBMIT, a new ssessment should be created but also gathering Customr data (Case ticket number and Customer name).

 

Is there a way to gather Customer and Case information in that Public Survey?
