Root Cause Analysis PDF Removed

In the Problem Module we used to be able to click on Generate RCA to create a Root Cause Analysis Report using selected fields in a PDF format. This would then be added as an attachment to a Problem Record. If you generated another RCA, it was attached as a second attachment (and so on).
Lately, we noticed that the original RCA gets removed and overwritten with the latest one.
How do you retain all RCAs generated as Attachments to the PRB?

is that an OOTB UI action and logic to generate report and add attachment?
I couldn't find any.
If it's custom then check if any developer updated that logic recently

Update on this:
We found that you are able to save multiple RCAs as attachments, providing the document is renamed with the added text added at the end instead of at the start.
RCA Report.pdf = what ServiceNow names the file
Initial RCA Report.pdf = the system will overwrite this if you create another one
RCA Report Initial.pdf = the system will not overwrite this you create another one
