UI actions are sometimes not visible in CSM/FSM workspace

Hi All 

 

I am having issues when making UI actions visible in CSM/FSM workspace. Some times it doesn't work at all and the next day all of sudden you see the UI action button is now visible. I have tried created my own button which worked but however the second button I have created didn't work. I am really puzzled on what I am doing wrong? is it normal behaviour ? 
What do I do ?
System Definition > Ui action > create new UI action 

Configure Page> Action Bar configuration> UX Form Action Layout Items(change scope to CSM configurable workspace)>Add new (Add the button created)

Please let me know where I am going wrong and what am I missing.
