Hi all,

In the above UI Page, I've certain requirements to be done: 

1. In the action field - I have three values "Add", "Modify" and "Delete": If the associated case does not have any Request created - show only Add option, else the other two options.

2. When Add is selected: New Diversion Number should be shown else, it should be hidden.

3. In Case of Modify - the already entered Diversion number during Add should be shown.

3. On click of enable, it should create an Request and RITM unto the case created,

Any help would be helpful.

What have you already tried? Is there a specific issue you are struggling with or do you not know where to start? If you have no experience with UI pages (which is what it sounds like), you will likely not be able to implement this on your own with just some help from the Community. Before dealing with complex use cases, take the time to learn about UI pages in general. Get appropriate training, then look at the examples from the base system, then try to build simple UI pages on your own, then move on to advanced use cases.
