5 Whys RCA technique template

I desire to incorporate the format/template of the 5 Whys RCA technique into the Root Cause Analysis Type Problem task form. As indicated in the provided link, although the 5 Whys template exists, it has not yet been integrated into ITSM.

You could check with ServiceNow to see if this is somewhere (high) on their backlog. It would be unfortunate to put a lot of effort in it, when it's going to be an OOB feature soon.

Do you have that template on your instance, or isn't it there at all? If you have it, just check on its functionality and rebuild it for problem. If you don't have it, try implementing it on your PDI for reference. You will need to build it yourself if you need it now, because it's simply not there at the moment and you can't easily reuse things that are build for other modules.
