Action: Start Flow for RITM

I have created a custom action to start a flow for a newly create RITM within my master flow. It tests fine however, when launched from the master flow, I get the following error:

Can you clarify my below question
Are you creating RITM from Flow action "Create Requested Item" Script instead of Submitting Request ?

I am creating a new RITM and closing the existing RITM if a new existing software is suggested as an alternative to the request for new software.

when you say you are creating a New RITM you were submitting the Request by using cart functionality ?
or directly gliding the sc_req_item table and inserting the record ?

the original request comes from a catalog request from employee center.
