Activity Properties: Create Task Template not filling form

Hello, everyone. 

I am working on a task where a template should be auto populating two values when a Committee Review form is generated. While the Committee Review form does generate, the values are not populated. 

Any ideas how I can fix this so it populates correctly?

Assuming you are applying this template manually at the state. These values exit in the Order table if used manually. Try to use this template at any other state and it is an issue with one group or all?
