Add attachment through Mobile App using quick action

Hi,

I am working on a solution where I want to create incident for an asset assigned to me from Servicenow Mobile application. I am using an Action Item and Action function to create the incident. The incident is created for the same asset. But I am not able to add attachment this process. I saw an action item parameter type Attachment but not sure how to use it. Also Image upload function is not working in this purpose. 

Any help will be appreciated.

this is for showing attachment on applet. I want to add attachment while I am creating an incident through mobile app.
